NEW PUPPY AGREEMENT/HEALTH GUARANTEE
2006
______________________________________________
CONGRATULATIONS ON YOUR NEW DACHS-N-PUPS PUPPY!!
A puppy is a lifetime commitment of teaching, nurturing, care and love. It is the new owners’ responsibility to make sure your puppy receives all health care and vaccines after acquiring in a timely manner to prevent any puppy hood illnesses. Your new puppy will have received worming treatments, a minimum of one vaccine and will be current through the date of receiving.
Your new puppy is sound and healthy, to the best of DACHS-N-PUPS knowledge at time of receiving. If there are any problems that we are aware of, will be noted in the "New Puppy Agreement/Health Guarantee" and made known to the new owner prior to receiving.
DACHS-N-PUPS requires you to take your new puppy to a veterinarian, of your choice, for an initial health evaluation within 3 days from date of receiving for our "New Puppy Agreement/Health Guarantee" to be in effect. Should your puppy be diagnosed with a serious health problem at time of evaluation, DACHS-N-PUPS is to be notified immediately. DACHS-N-PUPS will not accept injuries occurred after leaving our premises as any excuse for a refund. If you have made a mistake in taking on the responsibility of owning a new puppy regardless the reason, you may return the puppy to us at anytime. There will be NO REFUNDS OR EXCHANGES in this instance.
DACHS-N-PUPS is not responsible for any veterinarian costs after said 3 day period.
This document serves as DACHS-N-PUPS "Written Health Guarantee". Should your puppy be diagnosed with a "detrimental" genetic defect within one year of birth, DACHS-N-PUPS will replace said puppy with a puppy from a future litter (when available) of equal purchase value, or refund $250.00. This will be at DACHS-N-PUPS discretion. A detrimental genetic defect is determined as one that is life threatening, may cause imminent death, or the need for euthanasia. DACHS-N-PUPS must receive all veterinarian documentation regarding a diagnosis preceding death, for evaluation by our personal Veterinarian. DACHS-N-PUPS reserves the right to have said puppy taken (at our expense) for a second veterinary evaluation. DACHS-N-PUPS requires a Necropsy (animal autopsy) for verification of cause of any death within the guarantee period.
If this DACHS-N-PUPS puppy is being sold/purchased as a family companion with "limited" AKC registration, it is our policy of responsibility that said puppy shall be spayed/neutered before the age of 7 months. If the spay/neuter agreement is not fulfilled, the ”New Puppy Agreement/Health Guarantee" becomes null and void.
**AKC REGISTRATION PAPERS WILL BE MAILED TO YOU UPON OUR RECEIVING VET CERTIFICATION OF SPAY/NEUTER.
Please print a copy of this agreement for your records.
